Clinical profile in arterial disease of the lower limbs

It organizes the main clinical and anatomical elements and provides an indicative evaluation path. It does not calculate a risk percentage or formulate an individual indication for revascularization.

Uso professionale. The result is an indicative summary to support the clinical review. Integrate physical examination, ABI/TBI, imaging, comorbidities, preferences, and vascular team assessment. TASC/GLASS anatomical classes are not automatically inferred.
1. Clinical presentation
Frame the clinical picture before interpreting the percentage of stenosis.
Diabetes can coexist with atherosclerosis; it is not a mutually exclusive etiology.
Impatto e segni associati
Perfusion and wound parameters (if available)
Che cosa significa WIfI?

WIfI significa Wound, Ischaemia and foot Infection: classifies wound, ischemia and foot infection (grades 0–3) and combines them into a clinical stage 1–4, associated with the risk of limb loss and the possible benefit of revascularization.

It applies above all to chronic ischemia threatening the limb. The stage does not alone determine the treatment: it must be interpreted with anatomy, patient risk and multidisciplinary evaluation. This field accepts the stadium already evaluated; it does not calculate the class.

High/non-compressible ABI may be unreliable, especially in diabetes and nephropathy: consider digital pressure, TBI, TcPO₂ and Doppler tracings.
2. Sede ed estensione documentate all’imaging

It is a historical anatomical classification that can help describe the lesion, but does not alone select the technique: the current choice also depends on the clinical picture, risk, complete anatomy, preferences and resources/experience of the team. Enter the class already assigned and specify the territory in the report.

TASC II supplement: updated classification and strategies

TASC II scheme: classes A–D of aortoiliac and femoropopliteal lesions and historical orientation on treatment strategies.

Che cosa significa GLASS?

GLASS (Global Limb Anatomic Staging System) stadia l’anatomia infrainguinale nella chronic ischemia threatening the limb. Consider the target arterial pathway (Target Artery Path), the femoropopliteal and infrapopliteal districts and a modifier of the foot circle; stages I–III express increasing anatomical complexity and are used to estimate the expected patency of the revascularization path.

It must be integrated with the severity of the limb (WIfI) and the patient's risk (PLAN scheme). It is not intended for claudication or asymptomatic PAD alone and does not by itself indicate which procedure to perform. Enter a stage already assigned by the vascular team.

TASC and GLASS should only be entered if already assigned by a specialist: the calculator does not derive them from incomplete data.
3. Factors useful for planning
For claudication, revascularization is evaluated after insufficient response to medical therapy and structured exercise. The presence of anticoagulant or high bleeding risk modifies the antithrombotic choice.

How to read the summary

The strategy depends on the ischemic picture, the complete anatomy, the patient's risk and the expected benefit. In case of claudication, the initial treatment is generally conservative and oriented towards reducing cardiovascular risk and improving walking ability; a procedure is discussed if the symptoms remain limiting despite an adequate course. In case of chronic ischemia threatening the limb, a timely vascular evaluation is needed to plan whether and how to revascularize.

The choice between endovascular, surgical or hybrid approach is individual. The tool does not assign a TASC or GLASS class and does not automatically select the technique. Spinal cord stimulation and sympathectomy/denervation are mentioned only as niche options to be discussed in selected cases without the possibility of revascularization; they are not recommended as a standard alternative.
